To refill your Ballybane Pharmacy Prescription please complete form below
To refill or order your prescription please complete the refill form. Indicate in the additional notes information box your preferred pick-up day and if you would like to receive a text or phone call when your refill is ready. Please allow 48 hours for your refill to be made when selecting your preferred pick-up day.
When ordering a prescription please include your email and mobile phone number. Please complete as many fields as possible to assist us in completing your request. Indicate if the prescription is NEW and being emailed by your GP; NEW with a photo of the original prescription or a REPEAT on file. Select whether you require all or some items on the prescription and if some items, please list which ones. If you require the exact same as last month then please state “Exact same”. If you have a paper prescription from your GP, please bring the original prescription with you. Thank you.
